Family Home Visitor

Polk County Public Health

Kathy Reitmeier, RN, PHN, CLC

Kathy has 33 years of nursing experience and has had the opportunity to care for individuals across a wide range of healthcare settings, including hospital care, long-term care, same-day surgery, and now public health. These experiences have shaped her strong clinical foundation and deep commitment to providing compassionate, patient-centered care.

For the past 13 years, Kathy has worked as a public health nurse, where she discovered her true passion by serving families in the community. As a family home visitor, she supports moms, babies, and families during some of the most important stages of life. Her work focuses on education, encouragement, and connecting families with resources that promote healthy growth and development. She has also worked in WIC and in sexual and reproductive health, allowing her to provide well-rounded, holistic support tailored to each family’s unique needs.

What Kathy enjoys most about this work is building meaningful relationships and empowering families with the knowledge and confidence they need to thrive. Being welcomed into someone’s home is a privilege she takes seriously, and she strives to create a supportive, nonjudgmental environment for every family she serves.
