The MN DHS Office of the Medicaid Medical Director (OMMD) is hosting another round of listening sessions to share Transforming Maternal Health (TMaH) model updates and gather feedback on provider selection, model implementation, and preparation for infrastructure and value-based payments.
Discussion topics will likely include: • Medicaid billing and reimbursement challenges, • data integration needs, • collaboration between community and hospital-based providers, • screenings and wrap around care for perinatal substance use disorder, mental health, chronic disease, oral health, and health-related social needs (HRSNs)
Please register for only one session. We acknowledge many of you wear various hats. Please register for the session that reflects your primary role.
We will be deidentifying, summarizing, and sharing results back with all participants.
We kindly ask partners not to register as a listener in other sessions to protect the creation of safe spaces for partners to share honest feedback.
Follow-up survey: There will also be a follow-up survey to provide additional feedback and for those unable to attend listening sessions.
