NHSA’s Women’s Health Series: Addressing Chronic Conditions in Pregnancy and Postpartum

The learning objectives for this webinar are:
1. Identify a minimum of three risks to women’s/pregnant and postpartum people’s health associated with chronic conditions present during pregnancy and the postpartum period.
2. Discuss respectful strategies to support individuals in creating interventions to improve health status related to identified chronic conditions.
3. Consider how implementation of the AIM CCI Community Care to Address the Management of Chronic Conditions During Pregnancy and the Postpartum Maternal Safety Bundles can contribute to improving the health status of pregnant and postpartum people affected by Chronic Conditions.
