Brian Grahan

Brian Grahan, MD, PhD is the founder and Medical Director of Hennepin Healthcare’s (HH) office-based addiction treatment clinic, Director of HH’s substance use-related ECHO initiatives, and primary care provider in the Coordinated Care Center. He completed a combined internal medicine-pediatrics residency and addiction medicine fellowship at the University of Minnesota, as well as graduate and post-graduate work in outcomes measurement and behavior change at the University of Wisconsin-Madison and the Minneapolis VA Medical Center. Dr. Grahan seeks to create systems of care that reduce the impact of substance use disorders on individuals, their families, and their communities. Mentoring primary care and other healthcare providers in addiction medicine is a core part of these efforts, which has been supported by longitudinal funding from healthcare systems, accountable care organizations, and the State of Minnesota.
